Why this exists

Template packs are written for nobody.

Most practice documents are bought as a bundle, lightly edited, and then quietly tolerated for years — because rewriting them means sitting down with a blank page after a full caseload.

The cost is real but invisible. An intake that asks the wrong questions makes your first session slower. A consent that reads like a contract sets the wrong tone before anyone has said anything. A progress-note template that doesn't match how you actually think turns documentation into translation work, every single time.

What I draft

The documents.

Intake and history. Built around what you actually need to know before session two, in the order a person can answer it.
Consents and practice policies. Informed consent, telehealth consent, cancellation and fee policies, communication and confidentiality language — written to be read by a human, not just signed.
Treatment plans. Templates that fit your modality instead of fighting it, and produce something a payer will accept.
Progress notes. Note structures matched to how you think in session, so documentation takes minutes instead of an hour.
Client-facing material. Psychoeducation handouts, between-session practices, aftercare and discharge letters.

Delivered as editable documents, formatted and ready to use. If you want them designed rather than merely formatted, I'll do that too — as a one-time build, not an ongoing arrangement.

In two languages

English, or both.

Plenty of practices need good English documents and nothing else, which is why this stands on its own.

If you also see Spanish-speaking clients, the same documents can be produced bilingually, with the Spanish verified line by line and delivered with a signed statement of clinician review. That's the Spanish Materials service — the two are often bought together.

Scope

What I do and don't claim.

These are clinical drafts, not legal instruments. Consents, privacy notices and policies carry legal and regulatory weight that varies by state, licence, and payer. I draft them clinically — clear, accurate, and appropriate to the work — and you should have anything with legal consequence reviewed by your own attorney or professional liability carrier before you use it.
